Spanish/English Bilingual Help Me Grow Service Coordinator
Minimum requirements: Associate's Degree in major field of study related to early childhood (psychology, nursing, early childhood or Pre-K education, human development, family and community health, social work, etc.) OR a Bachelors Degree containing 12 quarter hours of 18 semester hours from these fields. Fluency in written and spoken Spanish. Have a service coordination credential issued by Ohio Dept. of Health (or able to obtain within 18 months of hire). Manage multiple tasks, organize excellently; work independently within a team. Have a valid Ohio driver's license and liability insurance. Prefer two years experience working with children birth to 3 years old.
Preference: Experience with Help Me Grow
General responsibilities: Maintain a caseload of 40-45 clients for whom the Service Coordinator is the primary contact for the program. Ensure at least one successful contact for each participating family every 30 calendar days by making phone calls and home visits at times convenient for the family. Coordinate and ensure completion of all required eligibility determination procedures within 45 calendar days of referral. Coordinate completion of all required initial IFSPs (Individualized Family Service Plans) and reviews. Together with the IFSP team, determine which services are necessary and list them on the “Services and Supports” section of the IFSP. Coordinate service delivery in accordance with Part C policies. Solicit information relevant to the assessment of the child from service providers, and together with the IFSP team, use information gathered to develop outcomes and strategies. Provide prior written notice of all IFSP meetings to all parents and providers at least 14 calendar days prior. Provide a copy of the IFSP to the family (in their native language) and service providers within 10 business days of the signature date. Coordinate completion of all required initial and ongoing assessments and screenings. Coordinate linkages between families and additional supports and/or advocacy providers. Ensure parents receive documentation of their rights in Help Me Grow at every IFSP review, and provide informed consent prior to provision of all services. Document all exceptional family circumstances that make it impossible to comply with required Help Me Grow timelines. Ensure all information and services are provided to the family in their native language. Ensure all client records are kept in a confidential and secure manner, with all required paperwork. Ensure accurate entry of all required data into Early Track within 30 calendar days of the event. Attend all required training in-service sessions, HMG and CRIS meetings, conferences, workshops etc. Complete skills inventory with clinical supervisor's approval and completion of a personal profile with validation on the Ohio Professional Development Network Registry.

Supplemental Services Program Case worker - Nepali speaking
Minimum requirements: Bachelors Degree or 2 years experience in a directly related profession Resourceful, pro-active problem-solving ability Fluency in English and Nepali Skilled in interpersonal and intercultural communication Strong organizational skills Experience in delivery of social services or employment services Working knowledge of MS Word and Excel, Internet and email Valid driver's license, appropriate insurance and good driving required (last only if transporting clients) Ability to travel (without notice) around Columbus in order to accompany clients Excellent (English) writing skills Professional demeanor

General responsibilities: Interview and assist clients with problems related to personal adjustments, finances, employment, food, clothing, and housing Compile records and prepares monthly reports Provide translation and interpretation as necessary for program clients May be required to visit clients in homes or other institutions Counsel clients regarding plans for meeting needs and aids client to mobilize inner capacities and environmental resources to improve social functioning Refer clients to community resources and other organizations May assist in informing clients about public benefit entitlements May work in collaboration with other professional disciplines Perform general receptionist duties as necessary Attend and contributes to program and agency meetings and trainings Other tasks as may be assigned
